Professional Experience

This tracker helps keep your professional experience hours organized, in line with the documentation required for Section 4 of the AASECT Certified Sexuality Educator® application. This section of the tracker can be downloaded separately and uploaded for this section.

This sheet will be used to log your professional and direct teaching experience. The AASECT website (linked above) describes what types of activities qualify for professional experience hours. For each experience, you will use a new row to list the institution or organization, your role, the start and end dates, a brief description of the activities, and the number of professional experience and/or direct teaching hours.

The two cells above the tracker will automatically tally your hours as you log additional experience. When you reach 1,000 hours of professional experience and 100 hours of direct teaching experience, the respective cells will change from red to green.

Supervision Experience

This tracker helps keep your supervision hours organized, in line with the information required for Section 8 of the AASECT Certified Sexuality Educator® Application.

For each date of supervision, you will list the date, supervisor name, and hours of group or individual supervision. The three cells at the top automatically track your total supervision hours. The respective cells will turn from red to green when you have reached a minimum of 5 hours of individual supervision, 20 hours of group supervision, and 25 hours of supervision overall.

This tracker does not need to be directly uploaded to the application form, but will be necessary for inputting your completed supervision hours and allowing your supervisors to complete their endorsement forms.

Digital Goods may be used only as expressly permitted in the Terms or these Rules; all other uses are prohibited (unless you have contacted the copyright owner and received prior, written consent to the contrary). The Terms and these Rules apply even if the price or payment due for a Digital Good is zero. If these Rules conflict with the Terms (or any other terms), these Rules control what you can do with Digital Goods, with one exception – if the webpage or other page in the Store or applicable Service for a specific Digital Goods item specifies a different number of devices or otherwise specifies particular rights or limitations that vary from these Rules, that webpage or other page will control as to the expressly conflicting statements, but only for that particular Digital Goods item.

Digital Goods are licensed solely for your personal, non-commercial use (which excludes use for promotional purposes), at a level customary for such use. Mending Connections, PLLC may stop distributing any Digital Good, or add to or reduce the capabilities for any Digital Good, at any time. You may lose access to or capabilities of Digital Goods, or have the nature of your access changed (e.g., lose access, if any), if:

(a) you violate the Terms or these Rules,

(b) you use or try to use Digital Goods in a manner inconsistent with the Terms, these Rules, or the limited license granted to you,

(c) you circumvent, or try to circumvent, any technical measures designed to limit access or enforce license limitations,

(d) you fail to pay a fee

(e) you fail to maintain a current payment method for your account,

(f) you change your country or region,

(g) you fail to maintain a current email address associated with your account,

(h) you fail to log into a given Service sufficiently frequently to refresh your Digital Goods electronic licenses as described in the Additional Service-Specific Rules below,

(i) we cancel or terminate the applicable Service or cease supporting the applicable device, or

(j) we lose our rights to continue to distribute the Digital Good to you, or to distribute it to you in the same manner. Except as may be permitted below, you may not transfer or resell any licenses to any Digital Goods. If you sell any device containing Digital Goods, the purchaser will not acquire any right to use the Digital Goods. We may use technologies to verify your compliance with these Rules.
All sales are final. No refunds will be issued.
Protecting you private information is our priority. The Statement of Privacy applies to www.MendingMyConnections.com and Mending Connections, PLLC and governs Data collection and usage. For the purposes of this Privacy Policy, unless otherwise noted, all references to Mending Connections, PLLC include www.MendingMyConnections.com. The Mending Connections, PLLC Digital Product site. Collection of you Personal Information
In order to provide you with products and services offered many connections, Pllc may collect personally identifiable information, such as your: First and Last name and email address
We do not collect any personal information about you unless you voluntarily provide it to us. However, you may be required to provide certain personal information to us when you would like to use certain products or services. This may include: (a) registering for an account; (b) sending us an email message; (c) submitting your credit card or other payment information when ordering and purchasing products and services. To which, we will use your information for, but not limited to, communicating with you in relation to services and/or products you have requested from us. We also may gather additional personal or non-personal information in the future.

Mending connections, Pllc collection uses your personal information to operate and deliver the services you have requested.
Many connections, Pllc may also use your personal identifiable information to inform you of other products or services available from many connections, Pllc and its affiliates.
Sharing Information with Third Parties
Mending connections, Pllc does not sell, rent or lease its customer list to third parties. Many connections, Pllc may share data with trusted partners to help perform statistical analysis, send you email or postal mail, provide customer support, or arrange for deliveries. All such third parties are prohibited from using your personal information except to revise the services to many connections, Pllc, and they are required to maintain the confidentiality of your information. Many connections, Pllc may disclose your personal information without notice, if required to do so by law or in good faith such action is necessary to: (a) conform to the edicts of the law or comply with legal process served on Mending Connections, PLLC or the site; (b) protect and defend the rights or property of Mending Connections, PLLC; and/or (c) act under exigent circumstances to protect the personal safety of users of Mending Connections, PLLC, or the Public.
Tracking User Behavior
Many connections, Pllc may keep track of the websites and pages or users visits within mending connections, Pllc in order to determine what many connections, Pllc services are the most popular. This data is used to deliver customize content and advertising with inventing connections, Pllc to customers whose behaviors indicate that they are interested in a particular subject area.
Automatically Collected Information
Information about your computer hardware and software may be automatically collected by many connections, Pllc. This information can include: your IP address, browser type, domain names, access times and referring website address. This information is used for the operation of the service, to maintain quality of service, and to provide general statistics regarding use of the Mending connections, Pllc website.
Rights to Deletion
Subject to certain exceptions set out below, on receipt of a verifiable request from you, we will:
Delete your personal information from our records; and
Direct any service providers to delete your personal information from my records.

Please note that we may not be able to comply with request to delete your personal information if it is necessary to.
This workbook was created in February 2026 and follows guidelines associated with sex therapy candidates starting in 2025.
